Student Medical History

The Student Medical History and Permission Form is required in therapeutic horseback riding to collect enough medical information to keep the client safe.  There are important points to document on this form such as; ensuring special precautions and contraindications to horseback riding are recognized, communicating information such as allergies and seizures, etc.    There is also a place to state the specific medical diagnosis of the client and a general medical condition list that helps RideAbility to make good decisions regarding activities.  The medical permission statement needs to be signed by a doctor and indicates that all known and pertinent medical information is recorded on this form.

The Student Medical History form is mandated to be updated annually for clients diagnosed with Down Syndrome and some other medical conditions.    The form is suggested to be updated bi-annually for many other medical conditions.   These updates include validating the form with a new signature by a doctor.
